Welcome to the ‘For Families: Phoning Your Long-Term Care Team Using The Huddle Tool’ eLearning course!
Have you ever wished your long-term care team could better understand what you wanted or needed, for you or your family member? You, as a resident or family member, have an important role to play in making decisions about care planning and delivery in long-term care. As a valuable member of the long-term care team, your input can improve care experiences and outcomes for you, for your family member, for staff, and for other care partners.
This course presents a tool for shared decision-making developed by the Ontario CLRI at Baycrest and University of Toronto Bloomberg Faculty of Nursing, in collaboration with families and residents in long-term care and other stakeholders.
Introduction
In this course you will:
- Learn how the Huddle tool is used to share your experience, situation, or need with your long-term care team over the telephone.
- Practice using the Huddle Tool so you can share information with your long-term care team more efficiently and effectively and help to improve care through shared decision-making.
This course will take you about 30 minutes to finish. At the end of the course, you will be able to download a copy of the Huddle Tool for you to use. After completing the course, you will be invited to volunteer to participate in a virtual interview for family members to share your experience with using the Huddle Tool. You can contact the study lead, Dr. Lisa Cranley, directly or the study team may email you (using the email provided upon registration) to invite you to share your experiences.
You will receive an Ontario CLRI certificate for completing this course.
