Implementing the Palliative Approach to Care: self-assessment gaps and priorities from 89 Ontario Long Term Care Homes

Presented at the Ontario Long Term Caregivers Clinician Conference reporting on comprehensive, interprofessional, self-assessment aligned to the HQO quality standard for palliative care completed by 89 homes in the CLRI Collaborative project.

Objective: The Collaborative Project pools regional and provincial resources to promote and sustain integration of a palliative approach to care in Ontario LTC homes.   This is aligned to the new FLTCA expectation that every resident be provided with care based on a palliative care philosophy.
