In North East Region, “Fundamentals – Fundamentals of Hospice Palliative Care” for all Health Care Providers

A certificate program for ALL health care providers and volunteers who wish to enhance knowledge and develop capacity related to hospice palliative care.

Fundamentals This certificate course focuses on developing the foundational skills and understanding of a palliative approach to care; having difficult conversations and meeting the needs of clients and their families across all domains of care when they are diagnosed or caring for someone whom has a progressive life-limiting/life-threatening illness. This course is for all providers across disciplines and teams as well volunteers and leadership personnel whom are caring for people with progressive life-limiting/life threatening illness.

Start Date and Duration: For Jan 2024-Dec 1st, For April 2024- Feb 1st. There will be 4 sessions spanning 3.5 hours.

In North East Region, “APPS– Advanced Palliative Practice Skills- for PSWs/HCA”

Whom have completed the Fundamentals of Hospice Palliative Care CORE program.

APPS is an eight-week program to further enhance the PSWs scope of practice as it relates to the concepts of Hospice Palliative Care. Participants will learn skills to enhance communication with the person, family, and team members, develop skills required for effective team functioning and self-care, and learn strategies for relieving common end-of-life symptoms

Start Date and Duration: For Jan 2024-Dec 1st, For April 2024- Feb 1st. There will be 3 sessions spanning 1.5 hours.

In North East Region, “CAPCE- Comprehensive Advanced Palliative Care Education” for RN, RPN, NPs 

Whom have completed the Fundamentals of Hospice Palliative Care CORE and ENHANCED program

CAPCE course embeds best practice standards and aligns with the Model to Guide Hospice Palliative Care. CAPCE focuses on the further development of the role of the nurse as a Hospice Palliative Care resource for the inter-professional team in long-term care homes, agencies, hospitals, primary care and community settings.

Start Date and Duration: For Jan 2024- Nov 1st. There will be 8 sessions each spanning 4 hours.

he PoET Southwest Spread Project team will present the exciting results of the PoET Southwest Spread Project (PSSP), which helps Ontario long term care homes align their habits, policies, and practices with Ontario’s Health Care Consent Act.

The results from PSSP suggest that this work holds promise for both long term care residents, and the health care system.
